Job summary

Vivie is seeking a Hospice Registered Nurse (RN) in Minnesota for a part-time role (16 hours per pay period). You'll deliver compassionate hospice care across patient homes, manage admissions and end-of-life care, and coordinate with the care team to develop and implement individualized plans.

You will supervise HHAs, participate in IDG discussions, and uphold quality standards while traveling to patient homes and providing on-call coverage on weekends and holidays.

Qualifications

• Current Minnesota RN license.
• CPR certification.
• Ability to read, write, and communicate effectively in English.
• Hospice, palliative, or end-of-life care experience preferred.

Responsibilities

• Provide comprehensive hospice care including admissions, routine visits, re-certifications, discharges, and end-of-life care; document visits and care plans.
• Supervise HHAs and provide guidance at least every 14 days.
• Communicate and collaborate with the IDG and care team; participate in care conferences.
• Demonstrate accountability and foster a positive, compassionate team culture.
• Travel to patients' homes within designated regions and be on-call for weekends, evenings, and holidays.
