Qualifications

• Hospice nursing experience required.
• 2 years hospice experience preferred or required as stated in the ad.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care with the care team.
• Communicates patient progress to the clinical manager and physicians.
• Maintains patient care progress records accurately and timely.
• Educates families on patient condition and needs.
• Makes referrals to other hospice disciplines when needed.
• Performs on-call responsibilities as assigned.
