Job summary

Amedisys is seeking a registered nurse for hospice care in Mystic, Connecticut. This PRN role offers a flexible schedule and the opportunity to support patients where they feel most comfortable—at home. Prior hospice or home health experience is required to ensure high-quality, compassionate care.

You will perform initial assessments, develop and revise care plans, educate families, and coordinate with the care team. Current RN license, CPR, and a valid driver's license are required.

Qualifications

• Current RN license in the state you will be assigned.
• One year of nursing experience preferred; RN new graduate program if less than one year.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes and revises the plan of care; maintains continuity with care team.
• Communicates patient progress to clinical manager, physician, and care team.
• Maintains patient care records with accuracy and timeliness.
• Records pain/symptom changes and outcomes as appropriate.
• Educates families on patient condition and needs.
• Makes referrals to hospice care disciplines as needed.
• Performs on-call responsibilities and services as assigned.
• Performs other duties as assigned.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
