Job summary

Heartland Home Health & Hospice LLC seeks a registered nurse to plan, organize and direct care in patients' homes and communities, emphasizing community health education and family involvement.

You will perform comprehensive assessments, develop and revise care plans, administer treatments, counsel patients and families, coordinate care as case manager, and collaborate with the hospice interdisciplinary team.

Qualifications

• Graduate of an accredited nursing program.
• 1–2 years recent acute care experience in an institutional setting, and 1–2 years in home care.
• Current RN licensure in the state and CPR certification.
• Must be a licensed driver with an insured vehicle.
• Bachelor’s degree with hospice care experience preferred.
• Management experience not required; may supervise hospice aides.
• Excellent observation, communication, problem-solving, and basic math skills.
• Able to lift, position and transfer patients; sustain long periods of walking/standing.

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments.
• Provide professional nursing care per the nursing process and Act under state Nurse Practice Act.
• Write and adjust plans of care; re-evaluate patient/family needs.
• Initiate and revise the plan of care as patient status changes.
• Use health data to determine nursing diagnoses and goals.
• Administer medications and treatments as prescribed; provide patient education.
• Coordinate care with physician and interdisciplinary team; act as case manager.
• Maintain accurate clinical notes and timely physician communication.
• Participate in on-call duties and ensure equipment/supplies are available.
• Supervise ancillary personnel as required and pursue ongoing professional development.
