Job summary

Atlantic Health in Morristown, NJ, is seeking a Registered Nurse for day and evening Flex shifts to support hospice services in Northern NJ. You will conduct comprehensive clinical assessments, determine hospice eligibility and appropriate levels of care, and collaborate with the Hospice Medical Director on care planning and admissions.

The role requires RN licensure, BLS, and a nursing degree. Preferred BSN with 1+ year of hospice or nursing experience.

Qualifications

• Graduate of an accredited Nursing Program.
• Current state licensure as a Registered Nurse.
• BLS certification.

Responsibilities

• Conduct comprehensive clinical assessments to determine hospice eligibility, prognosis, disease progression, functional decline, symptom burden, and patient/family goals of care.
• Obtain and review clinical documentation and collaborate with the Hospice Medical Director to determine hospice eligibility and the appropriate level of care, including General Inpatient (GIP) eligibility.
• Assess patient learning needs and evaluate progress toward goals.
• Meet with patients and families to explain hospice services, levels of care, and the admission process.
• Facilitate hospice consents, admissions, certifications, and initial care planning.
• Complete GIP admissions, including assessments, documentation review, physician collaboration, admission paperwork, and care plan development.
• Complete discharge assessments as indicated

Education
