Job summary

Atlantic Visiting Nurse is seeking an experienced Hospice Referral Specialist RN to join the Care Access Center in Morristown. This full-time daytime role focuses on assessing referrals, coordinating admissions, and educating patients and families about hospice services.

You will collaborate with physicians, case managers, and interdisciplinary teams to ensure timely initiation of hospice care and smooth transitions from hospital to home. A NJ RN license and hospice experience are required.

Qualifications

• Active RN license in New Jersey and hospice nursing experience.
• Strong assessment, prioritization, and documentation skills.
• Experience coordinating with physicians, case managers, and teams.

Responsibilities

• Assess referrals to determine appropriate level of care for hospice.
• Coordinate admissions and transitions from hospital to home or facility.
• Educate patients/families about hospice services and benefits.
• Collaborate with interdisciplinary teams and ensure timely documentation in Epic.
• Monitor regulatory requirements and payer guidelines related to hospice care.
