Job summary

Atlantic Health is seeking a Hospice Registered Nurse for full‑time, days, serving Sussex County and surrounding areas. Responsibilities include case management, patient assessments, developing and revising care plans, and interdisciplinary collaboration to ensure high‑quality end‑of‑life care.

The role requires RN licensure, BSN preferred, and BLS certification, with 1+ year in nursing or hospice desirable.

Qualifications

• Graduate of an accredited Nursing Program.
• Current state licensure as a Registered Nurse.
• BLS certification.
• Preferred: BSN; ASN must complete BSN within five years; 1+ year hospice/nursing experience.

Responsibilities

• Assumes responsibility for case management.
• Performs patient assessments and develops/implements plans of care.
• Revises care plans based on evidence‑based knowledge and patient progress.
• Participates in insurance reimbursement discussions and ensures appropriate utilization of services.
• Maintains visit productivity standards and completes admission and revisit documentation.
• Provides teaching to patients and families and evaluates teaching plans.
