Job summary

Atlantic Health System in Morristown, NJ seeks a registered nurse to assume case management responsibilities in hospice care. You will assess patients, develop and revise plans of care, coordinate with the interdisciplinary team, and ensure safe medication management.

The role requires RN licensure, BLS, and a nursing program graduate; BSN preferred. You will participate in IDT meetings, ensure service utilization, and educate patients and families.

Qualifications

• Graduate of an accredited Nursing Program.
• Current state licensure as a Registered Nurse.
• BLS certification.

Responsibilities

• Assumes responsibility for Case Management and develops care plans.
• Evaluates patient's eligibility for hospice care and monitors progress toward goals.
• Maintains medication orders and knowledge of pharmacology.
• Participates in IDT meetings every 2 weeks.
• Ensures appropriate utilization of services and adherence to scheduling.
• Supervises Home Health Aides and supports caregiving teams.
• Completes discharge and admission assessments and documents progress.
• Documents patient education tools used and reinforces teaching for patients and families.
