Job summary

TriHealth Hospice in Cincinnati is seeking a Hospice Aide/RN-support role to provide direct and indirect holistic care to patients and families. The position emphasizes compassionate, patient-centered care, independent field-based work, and collaboration with a multidisciplinary team.

Responsibilities include nursing assessment support, care planning with the team, and ensuring safety and comfort for patients across multiple counties in Ohio.

Qualifications

• Compassionate, confident working independently in home-care or field-based settings.
• RN license required; BSN preferred within 5 years of hire.
• Compact, professional nursing knowledge across biological, physical, behavioral, social, and nursing sciences.

Responsibilities

• Performs initial and ongoing assessment of patient and family; documents findings.
• Plans and updates care plans with patient/family and healthcare team; coordinates discharge planning.
• Maintains patient autonomy, dignity, and safety; responds to emergencies as needed.
