VIA Health Partners is a top hospice and palliative care provider seeking a registered nurse to coordinate and deliver patient care across interdisciplinary teams. You will perform nursing assessments, develop and update plans of care, and educate patients and families to promote independence and quality of life.

Ideal candidates have nursing licensure, CPR, and hospice experience, with CHPN preferred. The role emphasizes clear documentation, infection control, and collaboration with supervisors

Qualifications

• Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.
• A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
• Current CPR required; CHPN preferred.
• Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions.
• Experience in direct care of patients with life-limiting illnesses; hospice/palliative care preferred.

Responsabilités

• Delivery of patient care and coordination of patient/caregiver care.
• Conduct nursing assessments to guide care planning.
• Develops, implements, and updates the plan of care to ensure uninterrupted care and goals.
• Educates patients and caregivers; promotes independence.
• Communicates with IDG, patients, caregivers, and health professionals for continuity of care.
• Maintains detailed clinical documentation per VIA Health Partners guidelines.
