Job summary

Three Oaks Hospice, Inc. is seeking a compassionate Registered Nurse to administer skilled nursing care in patients’ homes, coordinate with the interdisciplinary team and families, and manage care plans for end-of-life care.

You will work across various cases, ensuring high-quality, patient-centered service in a supportive network. The role requires two years of RN experience, CPR certification, and strong communication and organizational skills.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication and organizational skills; high ethical standards.

Responsibilities

• Perform initial and ongoing comprehensive assessments and coordinate care with the IDT.
• Coordinate care with patient, family, and referring agency; manage care plans and outcomes.
• Document progress notes and maintain clear patient records.
• Provide on-call responsibilities and on-call service as assigned.
• Educate patients and families on disease process, self-care, and end-of-life care; train staff as needed.
