Treasure Coast Hospice is seeking a skilled RN in Fort Pierce, FL to provide comprehensive hospice nursing care under the Florida Nurse Practice Act. You will perform assessments, administer medications, and coordinate with the IDT to meet patient and family goals.

The role requires an active Florida RN license, CHPN within 12 months, CPR, and at least one year of clinical RN experience in a palliative or related setting.

Formación

• Current and active Florida RN license.
• Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
• Current and valid Florida driver’s license and automobile insurance.
• Current CPR certification required.
• Minimum of one year RN experience with physical assessment skills.
• Prior experience in oncology, home health, Hospice or palliative care preferred.
• Ability to handle sensitive issues with sound judgment, maintain boundaries and confidentiality.
• Ability to plan, organize and prioritize work.
• Self-directed and able to work independently in the field.
• Basic computer skills and EMR software experience preferred.
• Per Diem works at least one weekend per month.
• Attendance is a condition of employment.
• Participate in departmental initiatives and staff development.
• Contribute to program development strategies and collaborate with teams.
• Demonstrate effective communication with patients, families, and team members.
• Inspire staff to uphold mission and compliance standards.
• Meet or exceed standards for the position.

Responsabilidades

• Uphold Treasure Coast Hospice standards for patient visits and documentation.
• Motivate and lead self with integrity and energy.
• Apply hospice nursing skills: case management, crisis intervention, teaching, and thanatology.
• Perform comprehensive assessments of physical, emotional, spiritual and psychosocial needs.
• Develop and update care plans with the IDT and patient/family.
• Provide direct patient care including assessments, medications, wound care and ADLs.
• Administer medications per policy and document responses.
• Communicate relevant information to the IDT and participate in care plan reviews.
• Complete education requirements timely.
• Document variances, interventions and outcomes, including family education.
• Support compliance program and report issues as required.
• Supervise CNAs and LPNs and support organizational strategic goals.
