Job summary

Treasure Coast Hospice is seeking a Registered Nurse Case Manager for the Homes team in Stuart, FL. You will plan and deliver skilled nursing care, provide palliative and supportive services, and supervise CNAs and LPNs while ensuring compliance with Florida laws.

Responsibilities include comprehensive assessments, care planning with IDT, and medication administration. A CHPN certification within 12 months and a valid Florida license are required.

Qualifications

• Current and active Florida RN license.
• Must be CHPN within 12 months.
• Current Florida driver’s license and automobile insurance.
• Current CPR certification.
• Minimum of one year RN experience with physical assessment.
• Prior experience in oncology, home health, hospice or palliative care preferred.
• Must maintain confidentiality and professional boundaries.
• Ability to plan, organize and prioritize work.
• Self-directed and able to work independently in the field.
• Basic computer skills and EMR experience preferred.
• Attendance is a condition of employment.
• Provide program development strategies for Treasure Coast Hospice and lead staff toward our mission and values.

Responsibilities

• Uphold Treasure Coast Hospice standards for patient visits and compliant documentation.
• Motivate and lead self with confidence, energy and integrity.
• Perform comprehensive assessments of physical, emotional, spiritual and psychosocial needs of patients and families.
• Establish and update care plans based on goals of care in collaboration with the IDT and attending MD.
• Provide direct patient care including assessments, medications, wound care and ADLs.
• Administer medications per policy and document response.
• Coordinate with IDT and family; communicate pertinent information.
• Document progress, interventions and education provided to patients and families.
• Supervise CNAs and LPNs.
• Assist credentialing at facilities; maintain good standing with facilities.
• Support compliance program with federal/state/local laws.
