Job summary

Treasure Coast Hospice seeks a Registered Nurse Case Manager to provide skilled nursing care and palliative support in Fort Pierce, FL. The role requires Florida RN licensure, CHPN within 12 months, and strong clinical skills.

The position includes a salary range of $72,000 to $74,880 annually plus a $4,500 sign-on bonus. Responsibilities cover patient care, care planning, and collaboration with the interdisciplinary team to meet family needs.

Qualifications

• Active Florida RN license required.
• CHPN certification within 12 months of hire preferred.
• Must have valid Florida driver’s license and auto insurance.

Responsibilities

• Plan and deliver skilled nursing care per Florida Nurse Practice Act.
• Provide palliative and supportive care to patient and family.
• Document patient condition, interventions and outcomes accurately.
• Administer medications and manage care plans.
• Supervise CNAs and LPNs and ensure credentialing at facilities.
• Collaborate with IDT and attending physician to update care plans.
