Job summary

Treasure Coast Hospice in Fort Pierce, FL is seeking an RN for Nights On Call / Triage (Support Services). The RN will plan and deliver patient care, provide skilled nursing and clinical support across Martin, St. Lucie and Okeechobee counties.

Qualifications include an active Florida RN license, CHPN within 12 months, valid Florida driver's license, and BLS. Experience in oncology, hospice or palliative care is preferred; strong communication and documentation are essential.

Qualifications

• Active Florida RN license is required.
• CHPN certification within 12 months is required.
• Current and valid Florida driver’s license and auto insurance.
• Minimum 1 year RN experience with acute care, ER preferred.
• Experience in oncology, home health, hospice or palliative care is preferred.
• Must communicate clearly and maintain professional boundaries.

Responsibilities

• Uphold standards for patient visits and compliant documentation.
• Provide triage information and coordinate crisis care as needed.
• Lead and support case management and care plan review.
• Coordinate with physicians and staff to optimize patient care and resources.
• Educate patients and families about interventions and medications.
