Job summary

Treasure Coast Hospice is seeking an RN for Nights On Call / Triage in Fort Pierce, FL. The role covers triage by phone and in-person visits, with an emphasis on palliative and supportive care for patients and families across Martin, St. Lucie and Okeechobee counties.

An active FL RN license and BLS CPR are required, CHPN within 12 months is preferred. This is a full-time night shift. The position requires independent work, strong communication skills, and the ability to coordinate with the IDT

Qualifications

• Current and active Florida RN license.
• Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
• Valid Florida driver's license and automobile insurance.
• Minimum of one year RN experience with acute care, ER or charge nurse preferred.
• Experience in oncology, home health, hospice or palliative care preferred.
• Able to read, write and speak English; bilingual preferred.
• Excellent communication and interpersonal skills.
• Basic computer skills and EMR software experience preferred.
• Must hold a current BLS CPR certification.

Responsibilities

• Plan and deliver skilled nursing care per nursing process in accordance with policy.
• Provide palliative and supportive care to patient and family in Martin, St. Lucie and Okeechobee counties.
• Handle phone triage and in-person visits; participate in crisis care and IPU coverage as needed.
• Coordinate with IDT, physicians and staff to optimize patient care and resource use.
• Document interventions, assessments and outcomes in EMR and ensure compliance.
