Job summary

Treasure Coast Hospice is seeking a Registered Nurse Case Manager to coordinate skilled nursing and palliative care for patients and families in Stuart, FL. You will assess needs, plan care, and work with the IDT to implement and evaluate outcomes.

This full-time role requires Florida RN license, CHPN within 12 months, CPR, reliable driving, and strong documentation. You will supervise CNAs and LPNs, ensure compliance, and advocate for patient comfort and dignity.

Qualifications

• Current Florida RN license active.
• CHPN within 12 months.
• Valid Florida driver’s license and automobile insurance.
• CPR certification required.
• Minimum 1 year RN experience with physical assessment.
• Experience in hospice/palliative settings preferred.
• Must plan, organize and prioritize work.

Responsibilities

• Plan and deliver patient care per Florida Nurse Practice Act.
• Provide palliative and supportive care to patients and families.
• Supervise CNAs and LPNs.
• Collaborate with the IDT to update plans of care.
• Document care, interventions and outcomes; educate families.
