Hospice of the Western Reserve, now Reserve Care, seeks an experienced Registered Nurse to join our end-of-life care team in Ohio. You will provide skilled nursing services, coordinate the Plan of Care, and work with a transdisciplinary team to support patients, families, and caregivers.
The role requires Ohio RN license, CPR certification, a valid driver’s license, and ability to participate in on-call rotations.
Qualificações
- Licensed RN in the State of Ohio.
- CPR certification is required.
- Valid driver’s license with good driving record.
- Hospice/home health experience preferred; Palliative care certification desired within 2 years.
Responsabilidades
- Provide prescribed medical treatment and personal care to terminally ill patients in collaboration with the transdisciplinary team.
- Assess patient and family needs (physical, psychosocial, spiritual).
- Deliver intermittent skilled nursing services under physician direction.
- Monitor pain and symptoms and educate patient and family.
- Liaise with physicians and coordinate Plan of Care.
- Provide emotional support to patients and families.
- Refer to other services and follow up as needed.
- Maintain plans and reports related to hospice care.
- Participate in care conferences and discharge planning.
- Contribute to quality improvement activities.
