Job summary

Three Oaks Hospice is seeking a skilled Registered Nurse to administer hospice care to patients in their homes in Texas. You will coordinate with the interdisciplinary team and families, ensuring appropriate levels of care and timely updates.

The role emphasizes patient-centered care, compassion, and collaboration with a diverse care network to support patients and families at end of life.

Qualifications

• RN with state license and Texas driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification and strong computer skills.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs.
• Coordinate care with the IDT and family, ensuring plan of care updates.
• Educate patients and families about disease process, end-of-life care, and nutrition.
• Initiate preventive and rehabilitative nursing procedures as needed.
• Document progress notes and ensure timely communication with physicians.
• Handle on-call responsibilities and provide on-call care as assigned.
• Coordinate services and referrals based on acuity and family needs.
