Three Oaks Hospice in Texas is seeking an RN to administer skilled nursing care for patients in their homes within the Conroe/Willis area. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, ensuring timely and appropriate hospice services.

Requirements include an active RN license, CPR certification, and 2+ years of clinical RN experience; hospice or home health experience is preferred.

Competenze

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and a valid driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Mansioni

• Administer skilled nursing care to clients in their residence.
• Coordinate care with the interdisciplinary team and families.
• Provide education on disease process, end-of-life care, and self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Document progress notes and adjust care plans as needed.
• Coordinate services and prioritize needs with the IDT.
• On-call responsibilities and availability for assigned patients.
