Three Oaks Hospice is seeking a registered nurse to provide skilled nursing care to patients in their homes across the Bartow and Gilmer counties, Calhoun, and Cartersville areas. Qualifikationen

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and a valid driver's license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification; computer skills expected.
• Strong interpersonal and communication abilities.

Aufgaben

• Assess patients’ needs and coordinate care with the IDT and family.
• Administer skilled nursing care in patients’ homes and adjust care plans.
• Document progress notes and maintain clinical records accurately.
• Provide on-call coverage and respond to patient/family needs promptly.
• Educate patients and families about disease process, care options, and self-care.
• Collaborate with physicians and other providers to optimize care.
