Job summary

Agape Hospice Care is seeking a compassionate Registered Nurse to join our hospice team in the Bartow and Gilmer, Calhoun, Cartersville area. You will administer skilled nursing care in patients' homes, coordinate with the IDT, and support families through end-of-life care.

The ideal candidate has an associate or bachelor’s degree in nursing, an active RN license, CPR certification, and at least two years of clinical experience; hospice or home health exposure is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer and Microsoft Office suite preferred.
• Strong interpersonal and communication skills and ability to support patient and family.
• Demonstrate high ethical standards and compliance.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team and referring agency.
• Provide education to patient/family about disease process, end-of-life care, nutrition, and self-care.
• Prepare clinical and progress notes documenting patient progress.
• Coordinate services and prioritize needs with IDT members.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Determine scope and frequency of services based on acuity and needs.
• Respond to on-call responsibilities as assigned.
