Job summary

Three Oaks Hospice, Inc. is seeking an experienced Registered Nurse to provide skilled nursing care in patients' homes in the Greenville, Terrell, and nearby areas.

You will coordinate care with the interdisciplinary team, educate families, and ensure timely, compassionate end-of-life support. The ideal candidate holds an RN license in Texas, CPR certification, and at least two years of clinical nursing experience, with hospice or home health preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience.
• Strong interpersonal, communication, and organizational skills.
• Commitment to ethics and compliance in clinical practice.

Responsibilities

• Conduct initial and ongoing assessments of patient needs and ADLs.
• Coordinate care with the interdisciplinary team and family.
• Educate patients and families about disease process and end-of-life care.
• Initiate appropriate nursing procedures and document progress.
• Coordinate all patient/family services and refer to other services as needed.
• Provide on-call support to patients and families as assigned.
