Job summary

Three Oaks Hospice, Inc. in Texas is seeking a compassionate RN to administer skilled nursing care at patients’ homes. You will coordinate with the interdisciplinary team, patient, family, and referring agency to ensure timely, appropriate care and to maintain the care plan.

On-call duties are included as assigned. The role requires an accredited nursing degree, an active RN license, CPR, and 2 years of clinical RN experience (hospice/home health preferred).

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient use of computers; MS Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Administer skilled nursing care for clients in their residence; coordinate with the interdisciplinary team and family.
• Coordinate care with referring agency and IDT; adjust plan as needed.
• Provide patient/family education on disease process, end-of-life care, nutrition and self-care.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Prepare clinical notes and document progress toward goals.
• Assess caregiver ability and determine scope and frequency of services.
