Job summary

Swan Hospice in Central New Jersey is seeking an On-Call Registered Nurse (RN) to respond to patient census and family needs, including referrals and community inquiries outside office hours.

You will provide urgent clinical and psychosocial interventions, manage pain and symptoms, and collaborate with hospice and external providers to optimize outcomes, with independent crisis assessment and consulting the Medical Director and on-call administrator as needed.

Responsibilities

• Respond to hospice patient census and family needs outside office hours.
• Provide urgent clinical and psychosocial interventions and pain/symptom management.
• Collaborate with hospice providers and external teams to optimize patient outcomes.
• Exercise independent assessment and judgment in crisis situations, consulting the Medical Director as needed.
