Job summary

Swan Hospice is actively seeking a Registered Nurse (RN) to join our team in providing high-quality hospice care. The RN will manage a primary caseload, assess needs, and implement comprehensive nursing plans for patients in facilities and private homes.

The role emphasizes interdisciplinary collaboration, patient and family education, and supervision of aides/homemakers to ensure compassionate, compliant care. Licensure as an RN is required.

Qualifications

• Minimum of one year general nursing preferred with hospice, home health, medical, surgical or critical care experience.
• Experience with pain and symptom management preferred.
• Ability to work within an interdisciplinary setting.
• Understanding of Hospice philosophy and end-of-life care.

Responsibilities

• Oversees primary caseload of Hospice patients and provides skilled nursing care.
• Performs initial and comprehensive nursing assessments and updates care plans.
• Implements and reevaluates plan of care to meet patient needs.
• Teaches, supervises, and counsels patients, families, and caregivers.
• Collaborates with the Interdisciplinary Team to meet patient and family needs.
• Provides supervision to Hospice Aides/Homemakers.
