Job summary

Elevation Hospice of Colorado is seeking an experienced Registered Nurse to provide skilled nursing care for patients in their homes and to coordinate with the interdisciplinary team.

The RN will conduct comprehensive assessments, implement care plans, educate families, and ensure thorough documentation while delivering compassionate end-of-life care. A state RN license and CPR certification are required; two years’ RN experience preferred.

Qualifications

• RN license in current state required.
• Two years’ RN experience in clinical care setting preferred; hospice/home health experience is a plus.
• Current CPR certification required.
• Proficient computer skills; Microsoft Office experience preferred.

Responsibilities

• Initial and ongoing comprehensive nursing assessments for terminally ill patients in home/hospice settings.
• Coordinate care with the interdisciplinary team and family; manage care plan revisions.
• Educate patients and families about disease, self-care, and end-of-life care; provide staff training as needed.
• Prepare and maintain clinical notes; ensure accurate documentation.
• Provide on-call nursing services as assigned; participate in care coordination and referrals.
