Job summary

Elevation Hospice of Colorado is seeking an experienced Registered Nurse to administer skilled nursing care for patients in their homes, coordinating with an interdisciplinary team and families. You will assess needs, educate families, and ensure high-quality end-of-life care in a compassionate setting.

The role requires RN licensure, CPR certification, and at least two years of RN experience in a clinical setting.

Qualifications

• Licensed RN in the state.
• Current CPR certification.
• Two years of RN experience in a clinical setting; hospice/home health preferred.
• Valid driver’s license and reliable transportation.
• Proficient with computer use and basic Microsoft Office.

Responsibilities

• Assess patients’ physical, psychosocial, and environmental needs in the home.
• Coordinate care with an interdisciplinary team and patient/family.
• Educate patients and families about disease process, end-of-life care, and self-care.
• Provide on-demand nursing care and on-call coverage as assigned.
• Document progress notes and care plans to reflect goals.
