Job summary

Elevation Hospice of Colorado is seeking a qualified Registered Nurse to provide skilled nursing care in patients’ homes. The role coordinates care with the interdisciplinary team, patients, families, and referring agencies to ensure high-quality end-of-life care.

On-call duties are included; CPR certification is required. Qualified graduates hold an Associate or Bachelor’s degree in Nursing, possess an active RN license in Colorado, and have strong communication and organizational skills.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• CPR certification required.
• Proficient computer skills; MS Office experience preferred.

Responsibilities

• Administer skilled nursing care for clients in their residence.
• Coordinate care with the interdisciplinary team and family.
• Provide education to patients and families on disease process and end-of-life care.
• Implement and adjust the plan of care (POC) as needed.
• Document progress notes and communicate changes to the IDT.
• Perform on-call responsibilities and offer on-call services as assigned.
