Elevation Hospice of Colorado is seeking a dedicated Registered Nurse to provide skilled nursing care to patients at their homes or designated residence in Colorado. The role includes coordinating care with the interdisciplinary team and relatives, performing comprehensive assessments, and updating care plans as needs evolve.

The ideal candidate holds an RN license, CPR certification, and prior hospice or home health experience.

Qualifikationen

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred.
• Current CPR certification
• Proficient skill using a computer is required; Microsoft Office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
• Strong communication and organizational skills and high-standard of ethics and compliance

Aufgaben

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revisions to the plan as necessary.
• Consult with and educate the patient/family regarding disease process and end-of-life care.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate all patient/family services and prioritization of needs with the IDT.
• Inform physician and others of changes in patient needs and outcomes.
