Job summary

Elevation Hospice of Colorado is seeking a registered nurse to administer skilled nursing care for clients in their homes, coordinating with the interdisciplinary team and families to deliver compassionate end-of-life care. Responsibilities include comprehensive assessments, developing and updating care plans, educating patients and families, and delivering on-call coverage as assigned.

A Colorado RN license, CPR certification, and strong communication are required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years' experience as an RN in a clinical care setting; hospice/home health preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Demonstrate nursing practice skill, cope with family emotional stress, and respect diverse lifestyles.
• Strong communication and organizational skills with high ethics and compliance.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement the individualized Plan of Care and revise as needed.
• Educate patients and families about disease, care, and end-of-life options; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes showing progress toward goals.
• Coordinate services with the IDT and refer to other services as needed.
• Inform physicians of changes in patient status and outcomes.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability and ensure appropriate support systems.
• Communicate using current organization processes and tech.
• Apply admission criteria to hospice care and eligibility.
• Perform on-call duties as assigned.
• Other duties as needed.
