Job summary

Elevation Hospice of Colorado is seeking an experienced registered nurse to administer skilled nursing care in patients' homes within the Boulder area, coordinating with the interdisciplinary team and families.

The role requires 2 years of RN experience, active nursing license, CPR certification, and strong communication and organizational skills. This on-site role may include on-call duties as part of a compassionate hospice team.

Qualifications

• RN license in state of practice
• CPR certification current
• 2 years RN experience in clinical care; hospice/home health preferred
• Proficient with computers and standard software
• Strong interpersonal, communication and organizational skills
• Ability to work with patients and families in hospice care

Responsibilities

• Assess patients' physical, psychosocial and environmental needs.
• Coordinate care with the interdisciplinary team and families.
• Educate patients and families on disease, care plans and end-of-life needs.
• Document care and progress notes.
• Manage admission criteria and care plan adjustments.
• Provide on-call coverage as assigned.
