Job summary

Three Oaks Hospice, Inc. in Texas is seeking an experienced Registered Nurse to administer skilled nursing care for clients in their homes and coordinate care with the interdisciplinary team.

Responsibilities include conducting comprehensive assessments, developing and revising plans of care, educating patients and families, coordinating services, and providing on-call coverage within the Humble, Kingwood, and Cleveland areas.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office suite experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s physical, functional, psychosocial, and environmental needs.
• Implement the individualized POC and revisions to the plan as necessary.
• Consulting with and educating the patient/family regarding disease process, self-care, end-of-life care, nutrition and dietary needs; training staff as needed.
• Initiating appropriate preventive and rehabilitative nursing procedures.
• Preparing clinical and progress notes showing progress toward goals.
• Coordinating all patient/family services with IDT members.
• Using case management approaches and referring to other services as needed.
• Informing physicians of changes and evaluating patient/family response to care.
• Determining scope and frequency of services based on acuity and needs.
• Assessing caregiver ability to meet patient needs upon admission and during care.
• Communicating information using current organizational processes and technology.
• Applying criteria for admission and re-certification to hospice care to determine levels of care.
• Performing on-call responsibilities and providing on-call service as assigned.
• Other duties as assigned.
