Job summary

Three Oaks Hospice, Inc. is seeking a compassionate registered nurse to provide skilled nursing care to patients in their homes across the Humble, Kingwood, and Cleveland areas.

You will collaborate with an interdisciplinary team, coordinate care, and document progress while participating in on-call rotations as needed.

This role requires Texas RN licensure, CPR certification, and strong communication and organizational skills.

Qualifications

• RN licensure in Texas (or compact license)
• Two years RN experience in clinical care; hospice/home health preferred
• Current CPR certification
• Proficient computer skills; documentation in EMR or comparable system

Responsibilities

• Provide skilled nursing care to patients in their homes
• Coordinate care with the interdisciplinary team (IDT)
• Prepare and maintain clinical notes and progress documentation
• Participate in on-call responsibilities as assigned
