Enhabit Home Health & Hospice is seeking an experienced registered nurse to coordinate hospice services for patients in Georgia. You will review patient information at key care transitions and facilitate communication among physicians, patients, caregivers, and field staff.

The role requires a valid RN license, CPR certification, and minimum two years of nursing experience with hospice or home health. Strong tech skills and reliable transportation are essential for success.

Qualifications

• Graduate of an approved school of professional nursing with a valid clinical license.
• CPR certification is required.
• Minimum two years of nursing experience; at least one year in hospice or home health.
• Knowledge of federal, state and local laws governing home care operations.
• Strong communication and technology skills including mobile devices.

Responsibilities

• Coordinate hospice services delivered to patients according to standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
