Amedisys, part of the Optum family, seeks an RN Case Manager to coordinate compassionate hospice care, assess patients, and partner with the interdisciplinary team to develop tailored plans supporting each patient’s needs.

Responsibilities include patient assessments, care planning, documentation, pain management, patient and family education, on-call duties, and collaboration with the care team to ensure high-quality care in a home-based setting.

Formación

• Current RN license in applicable state(s).
• CPR certification required.
• At least 1 year of nursing experience.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
