Qualifications

• Requires Associate or Bachelor's degree in nursing.
• Minimum 1 year RN experience in home health or hospice.
• Minimum 1 year RN management experience.
• Current RN license (state(s) of assignment).
• Current CPR certification.

Responsibilities

• Provide leadership, supervision, and coordination of therapy services.
• Oversee clinical operations and ensure high-quality patient care.
• Review service requests and determine patient eligibility for home care.
• Coordinate referrals and admissions with intake; oversee scheduling.
• Oversee payor verification and precertification requirements.
• Review staff documentation and address missing or untimely items.
• Assist clinicians with care planning and set priorities.
• Evaluate patient needs and revise the plan of care as needed.
• Participate in on-call process and staff training.
• Address patient/family/physician concerns and support referrals.
