Qualifications

• Associate or bachelor's degree in nursing.
• Current RN license valid in assigned state(s).
• Current CPR certification.
• At least 1 year RN experience in home health or hospice.
• At least 1 year RN management experience.

Responsibilities

• Supports and cooperates with the director of staffing.
• Ensures quality of patient care and clinical performance of nurses and aides.
• Organizes clinical operations for the care center.
• Reviews service requests and coordinates patient referrals and admissions.
• Oversees scheduling and payor verification requirements.
• Monitors documentation accuracy and timeliness.
• Helps clinicians set care goals and develop the Plan of Care.
• Evaluates patient needs and updates the plan of care as needed.
• May provide direct patient care in emergencies.
• Participates in on-call processes and staff training.
