Requirements include a nursing degree, 1+ year RN experience in home health or hospice, 1+ year RN management, current RN license, and CPR certification. A sign-on bonus may apply for external candidates.

• Minimum 1 year of clinical RN experience in home health or hospice.
• Minimum 1 year RN management experience.
• Current RN license for assigned states.
• CPR certification is required.

Aufgaben

• Supports and cooperates with the director of staffing.
• Oversees overall quality of patient care and clinical performance of nurses and aides.
• Organizes clinical operations for the care center.
• Reviews service requests and coordinates patient referrals/admissions.
• Oversees scheduling and payor verification requirements.
• Ensures documentation is complete and timely.
• Assists in establishing Plan of Care with clinicians.
• Evaluates patient needs and revises plans of care as needed.
• May provide direct patient care in emergencies.
• Participates in on-call process.
• Trains staff to ensure quality care and fiscal responsibility.
• Addresses patient/family/physician concerns.
