The role requires a nursing degree, an active RN license in the state of hire, at least three years of nursing experience, and supervisory experience preferred. Strong communication and budgeting abilities are essential.

Qualifications

• Bachelor’s degree in Nursing or related field or equivalent work experience.
• Active RN license in the state hired to work or a multi-state license.
• Minimum of three years of nursing experience.
• One year of supervisory/management experience preferred.
• Active BLS for healthcare professionals from AHA or Red Cross; required before 2023.
• Previous oncology, hospice, or home health experience preferred.
• Geriatric care experience preferred for Nursing Home CMs.
• Ability to communicate in English (written and verbal).
• Ability to access homes and navigate stairs or tight spaces as required.

Responsibilities

• Oversees clinical staff to ensure patient care delivery is efficient and effective 24/7.
• Facilitates complex patient care conferences to meet plan-of-care outcomes.
• Directs weekly IDG meetings when applicable.
• Provides performance feedback and monitors accountability through audits and reports.
• Coordinates care between internal and external teams to meet patient needs.
• Administers direct nursing care as appropriate to maintain clinical competence.
• Manages staffing with the Director within budget and staffing models.
• Collaborates with Pharmacy, Infusion, DME, Hospice programs and Admissions to ensure needs are met.
• Ensures regulatory processes are updated and completed timely, e.g., care plans and physician orders.
• Participates in QAPI projects and addresses patient/family concerns.
