Job summary

Advance Hospice Care is seeking a Patient Care Manager to lead clinical operations, coordinate care, and mentor staff. The role emphasizes ensuring compliant, high-quality hospice services across the patient care continuum.

You will oversee scheduling, triage referrals, and lead interdisciplinary group meetings while upholding regulatory standards and driving performance improvements at the location.

Qualifications

• Current RN license in the state of employment
• Graduate of an accredited nursing program
• Minimum of 3 years direct patient care, with 2 years in hospice or home-based care
• Knowledge of hospice principles, standards and regulatory compliance (Medicare/Medicaid/JCAHO/ACHC)
• Ability to lead a diverse care team and adapt to emotional/clinical needs
• Strong interpersonal, organizational, and leadership skills
• Competency in clinical documentation and technology systems
• BSN preferred; leadership or supervisory experience in hospice preferred
• Experience with EMR and telehealth platforms advantageous

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services
• Manage visit scheduling and prioritize based on clinical judgment
• Triage referrals and medical concerns promptly and compassionately
• Lead IDG meetings and document minutes
• Mentor and train associates, ensure compliance
• Ensure care meets regulatory standards and company policies
• Support performance improvement, quality assessments, and operational efficiency
