Gentiva is seeking a Registered Nurse (RN) Clinical Liaison in Scottsboro, Alabama to ensure efficient intake and referrals, coordinate admissions, and support community awareness of hospice services. • Licensed RN with hospice experience.
• BSN is preferred but not required.
• CPR certification required.
• Valid driver’s license and automobile insurance.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice by reviewing medical records and communicating with physicians/families.
• Educate patients and families on hospice services and what to expect.
• Complete intake and admission processes meeting legal, clinical, and payer requirements.
• Serve as liaison between referral sources and hospice care team for smooth handoffs and rapid response.
