Continuum Hospice seeks an RN Clinical Director to lead an interdisciplinary team and ensure adherence to care guidelines. You will collaborate with the Program Director, Hospice Physician, and Community Liaisons to drive office excellence and patient growth.
The role requires strong leadership, emotional intelligence, and a deep commitment to compassionate end-of-life care, with a focus on quality, compliance, and staff development.
Qualifications
- RN with current state license to practice as a professional nurse.
- Minimum 3 years as a nurse, with 1 year direct hospice/home health/oncology care.
- At least 1 year of supervisory experience in hospice or home health.
- Ability to manage terminal care, crisis intervention, and family counseling.
Responsibilities
- Coordinate and oversee a spectrum of patient services in line with hospice policies.
- Provide guidance and support to a team of clinicians and staff.
- Conduct chart audits to ensure regulatory and ethical compliance.
- Develop orientation and in-service training for nursing/clinical staff.
- Evaluate staff performance and provide coaching to improve service delivery.
- Lead interdisciplinary team meetings to refine Care Plans for patients and families.
