Job summary

Moments Hospice is seeking a Hospice RN Clinical Manager/Team Director to lead an interdisciplinary team delivering compassionate end‑of‑life care. You will coordinate with nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers to ensure holistic patient care across our organization.

The role emphasizes autonomy, strong leadership, and effective communication with families and providers.

Qualifications

• Registered Nurse (RN) license required.
• Minimum 3 years of healthcare experience, hospice preferred.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
