Job summary
Hospice of the West is seeking a Registered Nurse Clinical Director to lead our team of clinicians, ensuring top-tier hospice care while aligning with federal and state guidelines. You will collaborate closely with the Program Director, Hospice Physician, and Community Liaisons to maintain office excellence and grow patient numbers.
The ideal candidate has 3+ years of nursing experience with supervisory responsibilities in hospice or related settings, strong leadership, and proficiency in EMR
Qualifications
- Bachelor's degree in nursing or health care preferred.
- Active Registered Nurse license in the state of practice.
Responsibilities
- Coordinate and oversee a spectrum of patient services, ensuring alignment with hospice program policies.
- Provide guidance and support to an interdisciplinary team of clinicians.
- Perform chart audits to ensure regulatory and ethical compliance.
- Assist in developing orientation and in-service training for nursing staff.
- Evaluate and uplift clinical staff performance during probation and annually.
- Lead interdisciplinary team meetings to refine patient and family care plans.
