Job summary

Heart to Heart Hospice in Evansville, IN is seeking an experienced RN Case Manager to deliver compassionate home-based care for patients with life-limiting illnesses. You will perform comprehensive assessments, implement and adjust the care plan, and coordinate with an interdisciplinary team to meet patient needs.

The role involves weekend 12-hour shifts (Fri-Sun, 8AM-8PM), on-call responsibilities, and documentation.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of RN experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement the individualized Plan of Care and revise as necessary.
• Collaborate with patient, family, physicians and care team.
• Attend IDG meeting and coordinate patient care planning.
• On-call responsibilities and timely documentation.
• Ensure compliance with laws, regulations, and policies.
