Job summary

Stillwater Hospice in Fort Wayne, IN, is seeking an RN Case Manager to coordinate nursing care for terminally ill patients at home. You will assess needs, implement care plans, and evaluate outcomes, reporting to the Clinical Team Leader. 32 hours per week, Mon-Fri with weekend rotations.

Requires RN license in Indiana, CPR certification, and 1 year of hospital or home health care experience. Competitive benefits and paid time off are included.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing preferred.
• Must have a current RN license to practice in the State of Indiana.
• Must hold CPR certification or obtain within the orientation period.
• Prefer one (1) year of recent hospital or home health care experience.
• Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
• Must be able to drive independently to all patient/family clients’ homes.
• Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of him/her, or to others in the workplace.
• Must be comfortable working with hospice patients with an emphasis on palliative care.

Responsibilities

• Manage and coordinate nursing care for terminally ill patients in home/hospice setting.
• Initiate, revise, and evaluate the interdisciplinary plan of care.
• Collaborate with LPNs, Home Health Aides, CNAs, and other staff as part of the care team.
• Participate in on-call and holiday rotations.
• Provide education and support to patients and families about hospice services.
