Job summary

Elara Caring is seeking a registered nurse to assess hospice patients and families, coordinate interdisciplinary care, and support bereavement services. You will review clinical records, communicate changes to the care team, and instruct patients and facilities on the Care Plan.

The role requires 1 year of clinical experience, RN license, and willingness to travel 50% to patient sites. Experience in hospice/home health is preferred and lifting up to 50 pounds is expected.

Qualifications

• Graduate of an accredited nursing program (Certificate/Diploma/Associate/Baccalaureate).
• Current state RN license.
• 1 year of clinical care experience.
• Hospice or home health experience preferred.
• 50% travel required.
• Ability to lift up to 50 pounds and move patients/equipment; reliable transportation.

Responsibilities

• Assess hospice patients and families to identify physical, psychosocial, environmental, psychological, social, and spiritual needs.
• Review documentation, clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations.
• Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physician, facility, and other care personnel.
• Counsel and instruct patients, facilities, and families in following the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain and symptom management to staff in alternate care settings.
• Provide appropriate support at time of death and perform bereavement assessments.
• Travel 50% as required for patient care and job tasks.
