Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join a mission-driven home health team in Missouri. You will assess patient needs, coordinate hospice care, and educate families while traveling up to 50% to patient homes with compassionate, high-quality care.

Reporting to the Clinical Team Manager, you’ll guide an Interdisciplinary Plan of Care and advocate for patient- and family-centered care across settings.

Qualifications

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Minimum 1 year of experience in a clinical care setting.
• Experience in a hospice or home health environment preferred.

Responsibilities

• Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs as evidenced by documentation and reports.
• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence health status.
• Communicates significant findings, problems, and changes in health condition to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings.
• Provides appropriate support at time of death and perform bereavement assessment.
