Job summary

Elara Caring is hiring a Registered Nurse to provide home health services across patient settings. You will assess needs, perform evaluations, and update care plans while coordinating with supervisors and physicians. Travel up to 50% and lifting up to 50 pounds are required.

Qualifications

• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• Current State RN license.
• 1 year of clinical care experience; hospice/home health experience preferred.
• Ability to travel up to 50% and to lift up to 50 pounds.
• Valid driver’s license and insurance.

Responsibilities

• Assess patient needs and develop the plan of care based on data collected.
• Perform initial evaluation and follow-up visits per patient and facility needs.
• Communicate health status changes to the supervisor and team.
• Counsel and involve patient and family in the care plan.
• Teach hospice philosophy and symptom management to staff.
